Family-friendly benefits that are easy to use. An HMO (health maintenance organization) health plan is the kind of plan that:  

• Lets you choose a Primary Care Physician (PCP) from our HMO network. Your PCP will probably be the doctor you see the most – for routine visits and care. They will also coordinate any other health care services you may need. And if you need to see a specialist, your PCP will need to make a referral.
• Has set copays for doctor visits, so you always know what you’re going to pay.
• Includes maternity benefits and prescription drug coverage which make HMO plans ideal for growing families.

Select HMO Plan
This is our lowest priced HMO that has its own Select network. Lower premiums and low out-of-pocket costs for covered services make this plan an economical choice. 

HMO Saver Plan
This mid-priced HMO gives you access to our entire HMO network and has a medical deductible for hospital and emergency services to help keep your premiums lower. 

Individual HMO Plan
This HMO offers great coverage with access to the entire HMO network. No medical deductible means immediate benefits for covered services.
